New patients
You can register at the practice by completing and signing a registration form available from reception, or you can also download our Registration Form.
Please ensure every page of the document is completed correctly, we will not accept incomplete forms.
If you choose to complete the form online, there are a number of ways of getting it back to us, you can;
* Print and complete the form and hand it in at reception.
*Post the completed form to us at Westfield Medical Centre, Malpas, Newport NP20 6EY..
*Or, if you have the facilities, you can print and complete the form, then scan and email it to Enquiries.w93045@wales.nhs.uk
Please contact us for further queries, but bear in mind the registration process can take up to 48 hours from us receiving your completed form.
Please note that we only register patients who are entitled to receive NHS treatment and whose permanent address lies within our catchment area.
